Standard Rights Release Form

Creative Brief Form
For large projects with more design needs than a rack card (ex: reports, proposals, PowerPoints, booklets, multiple-page documents). If your project requires multiple creative components, please fill out a Creative Request Form for each and the Creative Brief Form.

Events Creative Brief/Request Form
Please complete this form for events and materials associated with the event. Audio-visual needs, furniture, equipment, and such are all included in the request form.

Reprint Requests Form
Please fill out this form as early as possible for items including reprints of resource cards, brochures, rack cards, and flyers. Please note that the Marketing Team no longer manages laptops and projectors, which can be checked out by the IT Team.

Business Card Request Form
This form clarifies the necessary employee information required to request Mission business cards. When submitting your request, please allow 1 week for business cards.

Website Request Form
This form helps provide up-to-date and relevant information to our community partners, donors, volunteers, and more! When submitting your request, please allow 1 week for basic website edits and 2 weeks for new website content and navigation.

Email Request Form
This form clarifies your primary goals and target audience to deliver the most effective message. Given the high volume of emails the Mission sends, we will do our best to schedule your email as requested and follow up with any scheduling conflicts.

SWAG Order Form
This form is for requesting SWAG or Promotional Items for your department. When submitting your request, please allow at least 6 weeks before items are needed.

Social Media Request Form
This form is for requesting a social media post from the Mission's account. Either a corporate partner shoutout, inspiring story, call to action (volunteer, hiring post, etc.), or other. We will do our best to accommodate your request, but we cannot guarantee that your post will be approved.

Would you like a video for your program or a photographer to be present at your event? Fill out this form with all the necessary information so that we can better serve you and your needs.
