Food Services Request Form

This form should be used to request food for special events for programs. Food Services team will only provide the food, program staff will need to provide people to reheat, serve, and clean up. This form needs to be summitted at least three weeks prior to the event date. Food Services will need to approve the event and will notify you via email if your event is not approved.
